This form allows you to register your cat for a spay/neuter appointment at Good Mews. If you are needing to register more than one cat, you must make additional appointments. 

Please note: A non-refundable deposit of $15 per cat is required to reserve your appointment and is due at the time of registration. Deposits are applied to the total cost of the procedure. 

After submitting registration, you will be automatically redirected to view and sign the Medical Consent Waiver. This form must be signed prior to your appointment. 

Drop-off & Pick-up Times: Drop-off time on the day of your cat's procedure is 7:30 a.m. If you are running late, please contact us immediately to let us know. Pick-up time is at 2 p.m. We ask that you arrive on time to pick up your kitty. We are unable to board pets, and pick-ups after 2:30 p.m. will be considered late and result in an additional $20 late pick-up charge.
